Biography

Sarah Ghiocel received her Doctor of Pharmacy from Albany College of Pharmacy and Health Sciences in Albany, NY. She works as a pharmacist on the central Medicare wellness visit team at Community Care Physicians. Sarah is particularly interested in geriatric medicine, disease prevention, women’s health, and pre-and postnatal care.

Sarah has always been interested in science and medicine. She worked as a receptionist at an assisted living and skilled nursing facility during high school and community college. During this time, she built wonderful friendships with residents and grew to love the elderly population. Sarah enjoys helping them feel listened to and loves adding something as little as a laugh or smile to a patient’s day.

Sarah’s favorite part of her job, which she also thinks is the most important, is helping patients understand why their medication is necessary. This increases medication adherence, which results in better health outcomes for the patient. She also enjoys taking the time to make patients feel heard while tailoring a medication regimen to the patient’s individual needs. Sarah loves the philosophy of a patient-centered medical home that can coordinate healthcare across multiple settings so patients never get lost in the healthcare system.

Sarah is originally from Herkimer, NY, and moved to Albany for pharmacy school. She grew to love the area during that time and decided to call it home. She loves spending time with her three small children, her husband, and their dog. Sarah likes anything fiber-related, such as knitting, spinning yarn, and weaving. She enjoys all types of music, theater, animals, nature walks, reading, and hiking.
