Our clinicians are an integral part of your care team, led by your physician.
CCP’s central clinical services team is here to assist our adult and older adult patients take care of your health. Services offered by this team are particularly useful for patients living with multiple chronic conditions. Our providers, including doctors, nurse practitioners, physician assistants, pharmacists and care managers are all here as extensions of your primary care doctor’s office to ensure you are receiving the care and support you need. Some of these programs are available to all CCP patients, while others are available to those with Medicare and certain Managed Medicare programs as part of your insurance contract.
Annual Wellness Visits
If you’ve had Medicare Part B or Medicare Advantage insurance, you can get a yearly “wellness” visit to develop or update care plan based on your current health and risk factors. The yearly “wellness” visit isn’t a physical exam. The clinician performing your wellness exam will complete a health risk assessment to help you stay healthy and get the most out of your next visit with your doctor. Your visit may include routine measurements, review of medical and family history, review of medications and specialists, personalized health advice, advanced care planning, and cognitive assessments.
Transition of Care
Hospitalization and transitioning throughout an illness and recovery can be a vulnerable time for patients and their caregivers. We believe it’s vital to provide you support during all stages of an illness, especially with the known complexities and critical needs after a hospital stay. Our central services team assists our adult patients with transition from the hospital or skilled nursing facilities so you can better manage your health and avoid readmissions. We’re here to help during this time, which can sometimes be scary and confusing.
Care at Home
We believe one size does not fit all when it comes to caring for people living with chronic medical conditions. Your care should be personalized. While most medical care is provided in the doctor’s office, sometimes you may find it difficult to travel the office. Through CCP’s central clinical services team, your CCP primary care provider has additional clinical support and resources to offer you care in your home. This keeps your health on track and helps avoid things like unnecessary trips to the ER or Urgent Care due to unaddressed issues. As one care team, we deliver the care that fits all of your physical, mental, emotional, social, and spiritual health needs. That’s why your physician and our clinical services team are working together — for you.
Clinical Pharmacy
Pharmacist are an important part of your care team. Your primary care provider may engage our pharmacy team to assist you with the management of your medications. Our pharmacists don’t dispense medications. Instead they work with you to review and reconcile your medications; review side effects and special instructions; provide advice on over the counter (OTC) products; recommend generic or lower cost alternatives; help advise you if you miss a dose of medication; and help instruct you on the use of medical devices. If you are new to taking medications or are juggling multiple prescriptions, our pharmacists are useful members of your care team.
Care Management
Our care managers help our patients who have chronic or complex health problem to ease the pressure and confusion, improve your health, and make navigating healthcare easier. This team helps with coordination of care, rehabilitation facility tracking, management of conditions, and social and environment needs.
Behavioral Health
Our central clinical services team collaborates with the Integrated Behavioral Health team of Community Care Physicians. Our mind and body are connected, and your care should reflect that. CCP’s behavioral health team assists with behavioral changes and overall behavioral health including mental health, substance abuse, and lifestyle medicine.
