Chronic conditions and various complexities can make traveling to a clinical setting challenging, leading to patients not getting the care they need, and in many cases, adding to complications. CCP’s clinical services team offers timely care in your home that can help prevent high-cost visits to emergency departments and hospitals, as well as offer improved outcomes for our patients. Care at Home not only provides essential support for our patients but also serves as a valuable resource for your CCP provider. With additional resources at their disposal, your doctor can offer comprehensive care beyond the confines of their office. Although home visits do not replace all visits to the doctor, our team acts as a seamless extension of your primary care team, enabling us to bring specialized care directly to you for certain types of visits.
With these home visits, our team is able to deliver care, identify social barriers, conduct home safety checks, medication audits, develop caregiver partnerships, and more.

Patient Criteria for Home Visits
- You have a condition that makes leaving the home medically not advisable
- You have multiple chronic conditions which qualify for home care over a longer period of time
- You are negatively impacted when leaving the home
About a Care at Home Visit
During your visit, a clinician will discuss your health history and review your medications. This information will be shared with your doctor. We want to make sure your personalized care plan addresses your current health conditions and concerns. If you need more care at home, the program can also provide additional services, including:
- Care for chronic conditions
- Health screenings
- Medication management
- Social services support
- Urgent care
- Wellness exams
Home visits are good for you and your doctor
You remain comfortable and safe in familiar surroundings without missing critical visits with a provider. Your primary care provider has additional clinical resources to care for you to ensure you stay well and out of the hospital.
Am I Eligible?
CCP patients with Medicare Part B (Original/Traditional Medicare) are eligible for these enhanced services as a part of your insurance contract with CCP. If your provider feels you would benefit from this service, they will refer you to our team and we will contact you directly. Following your home visit, the CCP clinical services team will work with your CCP provider to coordinate the follow-up care that is necessary to meet your whole health needs. There is no charge to you for a home visit.
