Desiree Mollnow is a board-certified family nurse practitioner and a member of the Sigma Theta Tau International Honor Society, the American Association of Nurse Practitioners, and the Nurse Practitioner Association. She earned her Master of Science as a family nurse practitioner from the State University of New York Polytechnic Institute. Desiree began her undergraduate studies at the University at Albany, receiving a Bachelor of Science in human biology. Upon graduation, Desiree pursued a career in nursing. She received an Associate of Science in nursing from Samaritan Hospital School of Nursing and later earned a Bachelor of Science in nursing from Western Governors University.
Desiree has always been passionate about caring for others and knew she wanted to pursue a career in medicine at a young age. Before joining Community Care Physicians, Desiree worked as a registered nurse in various settings, including inpatient telemetry, outpatient allergy and asthma care, and, most recently, public health.
Desiree loves working with patients and providing them with the necessary resources and tools to reach their health goals. She believes viewing the patient as a whole and not just by their current disease process or condition is essential when providing quality patient care. What brings her the most satisfaction professionally is when patients have positive health outcomes and avoid preventable conditions. She has a special interest in diabetes, asthma/COPD, women’s health, and mental health.
Desiree was born and raised in the Capital Region, specifically Cohoes, NY. Outside of medicine, she enjoys spending time with family; she had two children during the pandemic, which kept her busy! Desiree also likes to travel, kayak, play volleyball, and do anything that involves being outdoors.
